Understanding Breast Ptosis — Anatomy and Causes
The breast rests on the pectoralis major muscle and is held in position by the suspensory ligaments of Cooper — fibrous bands that run from the skin through the breast to the chest wall fascia, providing structural support. Over time, these ligaments stretch and the skin envelope loses its elasticity. The breast tissue descends, the nipple-areola complex moves to a lower position, the upper breast pole becomes depleted, and the lower pole becomes pendulous.
Primary causes of breast ptosis in Indian women:
- Pregnancy and breastfeeding — the single most common cause; breast engorgement during lactation stretches the skin envelope; weaning causes deflation of glandular tissue; the stretched skin does not fully recover — particularly after multiple pregnancies and prolonged breastfeeding
- Significant weight loss — including post-bariatric surgery weight loss, which frequently causes massive loss of breast volume and elasticity leaving pendulous, empty breast skin
- Ageing — oestrogen decline at menopause reduces glandular tissue and skin elasticity; gravitational forces act over decades

Mastopexy Techniques
Periareolar Mastopexy (Donut/Benelli)
Suitable for mild ptosis (Grade I) only. A doughnut-shaped skin excision around the areola; the areola is reduced in size; the skin is gathered around the new areola. Scar: around the areola only — the most concealed scar pattern. Limitation: limited elevation for moderate to severe ptosis; purse-string suture may stretch the areola over time.
Vertical Mastopexy (Lollipop)
For moderate ptosis (Grade II). Periareolar skin excision + vertical incision from the areola to the inframammary fold. Reshapes the breast cone well; better elevation than periareolar. Scar: around areola + vertical line below — fades well.
Wise Pattern Mastopexy (Anchor/Inverted T)
For severe ptosis (Grade III). Periareolar + vertical + horizontal incision along the inframammary fold. Provides maximum elevation and skin resection; best shape for significantly ptotic breasts. Scar: around areola + vertical + horizontal — concealed within a bra/swimsuit.
Mastopexy Results — What to Expect
- Immediate (days 1–7): Significant swelling, bruising, and tightness; breast appears higher than expected (swelling pushes tissue upward); normal anxiety that the result appears “too high” — it will settle
- Weeks 2–4: Return to social activities and desk work; most of the bruising resolved; breast beginning to “drop” into its natural position
- Month 1–2: Breast shape settling; scars in early healing (red and slightly raised); compression bra continued during the day
- Month 3: Approximately 70% of the final shape visible; most of the upward displacement of initial healing resolved; scar fading begins
- Month 6: 85–90% of the final result visible; breast shape very close to permanent appearance
- Month 12–18: Final result with scars at maximum fading; complete assessment of outcome at this stage

When the breast is both ptotic AND lacks adequate volume (pseudoptosis or deflated appearance after breastfeeding), a breast implant is combined with mastopexy in the same procedure. This simultaneously restores upper breast fullness and corrects sagging. Technically more complex than either procedure alone — the competing forces of the implant and skin tightening require careful planning.
