What Is a Hydrocele?

The testicles are surrounded by two layers of tissue — the visceral and parietal layers of the tunica vaginalis — with a small amount of fluid between them for lubrication. A hydrocele develops when excessive fluid accumulates in this space, causing the scrotum to swell and appear enlarged. The fluid is typically clear, straw-coloured, and non-infectious. Most hydroceles are painless, though large ones may cause a heaviness or dragging sensation.

Hydroceles are extremely common — occurring in approximately 1–2% of adult men and in up to 80–94% of male neonates at birth (most resolve spontaneously within the first year of life). The condition is benign in the vast majority of cases but requires evaluation to exclude underlying testicular pathology such as epididymo-orchitis, torsion, or tumour.

Types of Hydrocele


1. Congenital (Communicating) Hydrocele — In Children

During foetal development, the testicle descends from the abdomen into the scrotum through a passage called the processus vaginalis. Normally, this passage closes before birth or shortly after. When the processus vaginalis remains open (patent processus vaginalis), peritoneal fluid from the abdomen communicates freely with the scrotal space — creating a communicating hydrocele. This type may vary in size during the day, becoming smaller in the morning (when the child is lying down) and larger in the evening. It is often associated with an indirect inguinal hernia. Most communicating hydroceles in children close spontaneously by age 2; those persisting beyond 2 years typically require surgical correction.


2. Non-Communicating (Simple) Hydrocele — In Adults

In adults, hydroceles develop when the tunica vaginalis produces excessive fluid or fails to reabsorb it adequately. There is no communication with the peritoneal cavity. This type does not change in size and is typically caused by trauma, epididymo-orchitis, torsion, or testicular tumour — or it may be idiopathic (no identifiable cause). Adult hydroceles that are symptomatic, enlarging, or cosmetically concerning are treated surgically.


3. Secondary (Reactive) Hydrocele

A reactive hydrocele develops in response to an underlying condition — most commonly epididymo-orchitis (infection/inflammation), torsion of the testis or appendix testis, or testicular tumour. Treatment of the underlying condition is prioritised; the hydrocele often resolves with the primary treatment. A reactive hydrocele associated with a testicular mass warrants urgent evaluation to exclude malignancy.

  • 3. Blood tests — tumour markers (AFP, beta-hCG, LDH) are checked if the ultrasound raises any concern about testicular malignancy

Hydrocele Surgery — Hydrocelectomy Explained

Surgical hydrocelectomy is the standard treatment for persistent or symptomatic hydroceles in both children and adults. It is a day-care procedure performed under general or spinal anaesthesia.

Surgical Techniques

Eversion technique (Lord’s or Jaboulay procedure) — the most commonly used technique for adult hydroceles. A scrotal incision is made, the fluid is drained, and the lining of the hydrocele (tunica vaginalis) is everted (turned inside out) and sutured behind the testis. This prevents fluid from re-accumulating in the space. The Lord’s procedure uses plication sutures without complete eversion, suitable for thin-walled hydroceles.

Excision technique — the tunica vaginalis is excised (removed) rather than everted, leaving only a small margin around the epididymis. Preferred for thick-walled, recurrent, or large hydroceles. Results in a very low recurrence rate.

Ligation of patent processus vaginalis (in children) — performed through a small inguinal incision, the patent processus vaginalis is identified and ligated (tied off) at the internal inguinal ring, eliminating communication between the abdomen and scrotum. The hydrocele fluid drains spontaneously. This approach simultaneously treats any associated inguinal hernia.
